Ontario has introduced significant changes to its immigration programs to address critical healthcare labor shortages. The Ontario Immigrant Nominee Program (OINP) has now made it easier for self-employed physicians to qualify for provincial nomination, removing previous barriers that prevented many from applying.
New Eligibility Criteria for Employer Job Offer: Foreign Worker Stream
Previously, applicants under the Employer Job Offer: Foreign Worker stream were required to have a job offer from an Ontario employer. This requirement made self-employed physicians, such as those billing through the Ontario Health Insurance Plan (OHIP), ineligible.
To resolve this issue and attract more medical professionals, Ontario has eliminated the job offer requirement for physicians. The new eligibility conditions include:
Registration and good standing with the College of Physicians and Surgeons of Ontario (CPSO) under one of the following certificate categories that permit patient care:
- Independent practice
- Academic practice
- Postgraduate education
Eligibility to receive payment for publicly funded health services under the Health Insurance Act, 1990
These changes took effect on January 27, 2025. It is important to note that this adjustment applies only to physicians (NOC 31100, 31101, and 31102). All other occupations under the stream must still meet the standard job offer requirement.
Changes to Express Entry Streams for Self-Employed Physicians
In another major update, Ontario has modified its Express Entry Human Capital Priorities and French-Speaking Skilled Worker streams to allow self-employed physicians to count their work experience towards eligibility.
Previously, self-employment was not considered valid work experience under these provincial immigration streams. However, the new regulations now allow applicants to use periods of self-employment if they worked in one of the following physician occupations:
- NOC 31100 – Specialists in clinical and laboratory medicine
- NOC 31101 – Specialists in surgery
- NOC 31102 – General practitioners and family physicians
Applicants must still meet the federal Express Entry eligibility requirements and choose between being assessed under one of two federal programs:
- Federal Skilled Worker Program
- Canadian Experience Class (CEC)
Importantly, the amendments apply only to self-employed physicians under these specific NOC codes. Other occupations remain subject to the existing work experience requirements.
These reforms mark a major step in addressing Ontario’s urgent healthcare labor shortages by making it easier for internationally trained and self-employed physicians to obtain permanent residency. By removing job offer barriers and recognizing self-employment, Ontario is ensuring that more doctors can contribute to the province’s healthcare system.
